SURF Application Details
To apply for an EAST Summer Undergraduate Research Fellowship (SURF), you must complete and mail or fax the required forms.
You may print the individual application pages directly from this Web site and fill them out by hand (where applicable). Several pages may be completed by typing directly into the Web page fields using your keyboard prior to printing, if you prefer.
Alternatively, you may download the complete application packet in either Word or PDF format, and either type your answers prior to printing (use Word format), or print the application and then fill it out by hand (use either format).
Individual Application Pages
The following are the required individual application pages. They are styled especially for printing from your browser [File > Print], and when printed will be optimized for paper. (Several pages may be completed by typing directly into the Web page fields using your keyboard prior to printing, if you prefer.)
- Application Cover Sheet;
- Personal Statement;
- Project Description;
- Work Plan;
- Agreement to Fulfill Program Requirements;
- Letter of Recommendation; and
- College Transcript.
